• Unrestricted Gift
  A gift that can be used where need or opportunity is greatest:
  I give to Children's Medical Center, Inc.,, a non-profit 501(c)(3) corporation, located in Boston, MA the sum of $_______* for its general purposes.
• Gift for a Specific Purpose:
  I give to Children's Medical Center, Inc.,, a non-profit 501(c)(3) corporation, located in Boston, MA the sum of $_______* to be used for [state purpose].
• Perpetual Endowment Fund
  A gift that benefits us in perpetuity:
  I give to Children's Medical Center, Inc.,, a non-profit 501(c)(3) corporation, located in Boston, MA the sum of $_______*, to be used as a permanent endowment, the income only from which shall be used for its general purposes (or describe the specific purpose, if desired).
• Residuary Bequest
  Leaves any remainder after all other bequests have been paid:
  All the rest, residue, and remainder of my estate, both real and personal, I give to Children's Medical Center, Inc.,, a non-profit 501(c)(3) corporation, located in Boston, MA for its general purposes.
• Contingent Bequest
  Takes effect only if a primary intention can't be met:
  I devise and bequeath the residue of the property, real and personal and wherever situated, owned by me at my death, to (name of beneficiary), if (she/he) survives me. If (name of beneficiary) does not survive me, I devise and bequeath my residuary estate to Children's Medical Center, a Massachusetts charitable corporation, located at 300 Longwood Avenue, Boston, MA 02115, to be used by the XXX department for (study, research and patient care) related to the subject of...
• Safety Language
  Language added whenever a gift is restricted:
  This designation represents a preferred use for these funds and is not an absolute restriction. Should the purpose for which this gift is intended cease to be effective or practicable at the time the funds are transferred, the Board is authorized to use this gift in an alternative way consistent with the general intent of this designation.

* Rather than leaving a sum certain, you can include language that refers to a percentage of your estate or a description of the property you wish to give.
